Job Summary

<span style="font-family:"Calibri",sans-serif">The Director, Care Transformation, is responsible for providing administrative leadership and care model strategy within the Care Transformation department at Prisma Health in order to develop care models to improve the quality and cost effectiveness of the management of populations.  Collaborates in care program development, implementation, evaluation, and outcomes of care model programs for population health management.  Interacts with Care Management leadership, primary care physicians, and other interdepartmental staff to collaborate in the development, implementation, and evaluation of specialized care models needed to manage disease processes of the populations that are served.  This position ensures the effective implementation of policies and procedures for Prisma Health in compliance with regulatory requirements, oversight of the operations, development of care model programs and integration of the necessary technology tools in order to carry out such programs.  

Supervisory/Management Responsibilities
Job has direct and/or indirect supervision of employees that may include final budget authority, hire/termination/disciplinary authority, performance appraisal responsibility. Job will be considered a member of the management staff with direct reports..
Minimum Education

Bachelor's Degree in Health Services

Minimum Experience

5 years of management/leadership experience in a variety of healthcare settings, working with physicians and administrative leaders
