Job Summary

Under the direction of a Registered Nurse, provides nursing care for patients.  Assumes responsibility for data collection related to patient status, on-going monitoring, measuring, and documentation of patient responses; provides patient/family instruction.  Performs additional patient care functions for which appropriately licensed. Functions as a part of the team, C NA, LHCTN and the Registered Nurse delegating assignments. LHCTN practices within their license and scope of service.

Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Education

Post high school education - LPN program.

Minimum Experience

2 years - LPN.

Required Certifications/Registrations/Licenses
LPN-LICENSED PRACTICAL NURSE - NLPN
